Screening Inventory of Personal History, Experience, and Needs 
In order to gain an understanding of the individuals who are invited to participate in the Community Project, each participant will be screened using an inventory that is designed to provide selected information about the individual. This information will be used to focus the planning efforts and strategies that will be utilized to facilitate each individual’s transition to independent living 
in their community. 

The areas covered in the screening inventory include the following: 

 
 
Money management 
[overall assessment of financial literacy] 

  Budgeting and saving 

  Banking; use of checkbook

Credit cards 

Employability skills

  Job search and applications

  Interviewing skills

  Resume

  Formal education/training

  Computer literacy; technological familiarity

Relationship skills

  History of significant relationships

  Family; close friends

  Community connections

  Communication skills

  Facility or difficulties expressing feelings 

Nutrition and food preparation

  Shopping preparation

  Meal planning

  Understanding of dietary and nutritional factors

Stress and anger management

  Coping skills and strategies

  Options for leisure/recreational activities 

Continuum of care

  Significant health history

  Knowledge of how to navigate various health care systems

  Experience with social service systems

  Special issues/challenges (e.g., addictions, disabilities)

  The Spiritual component
 

The format for the use and presentation of this inventory will include mini-classes 
and individual counseling/support sessions. These will begin at the initial enrollment 
and will then be tailored to the specific individuals.
